What we ask for on this website

The contact forms ask for your name, email, phone number, how and when to reach you, and a short message. A group inquiry also asks for your business name and team size. We use them to reply to you, and for nothing else. Please don't put medical details in a form.

Your health information

Your health history goes on our private patient portal, not in an email or on this website. We keep your records private and share them only as the law allows. An employer on the team plan gets a roster and an invoice, not visit details.

Your choices

You can ask what we have about you, ask us to correct it, or ask us to stop contacting you. Call (419) 555-0103 or write to hello@collingwooddental.example.

Changes

If we change this page, the new version appears here. The practice's owner replaces this sample text with its own policy.
